LIABILITY FORMS

In order to participate in Youth Ministry and Parish Events, each participating child and adult volunteer must fill out the required forms. These forms may be printed directly off the corresponding links, and turned into the Religious Education Office in the school. 

 

Any adult chaperone willing to participate as a driver is also required to fill out an automobile information sheet and return it before the day of the event.
